Please be advised that upon the receipt of a request for a medical record by a former patient by Iron Mountain Records Management:

An Iron Mountain Records Management Customer Service Representative will contact the requestor.
A summary of charges for the specific medical record will be sent to the requestor.
A Release of Information Authorization form will be sent to the requestor to fill out and sign.
Once payment has been received along with the signed Release of Information form, Iron Mountain Records Management will release and ship the requested medical record.
Notices will be mailed out, but it may be possible that interested parties do not receive the mailing, so that is why I placed this info on the web.
